Die Gemeinde liegt zwischen der Großen Kreisstadt Grimma (ca. 7 km) und der Großen Kreisstadt Wurzen (ca. 10 km) am Westufer der Mulde im sächsischen Hügelland. Weitere Gewässer auf Stadtgebiet sind das Altenhainer Wasser, der Kranichbach und das Mutzschener Wasser, die alle in die Mulde münden. Trebsen liegt auf flachem Terrain bei etwa 122 m ü. NN, wobei Höhenunterschiede bis auf 135 Meter über Normalnull auftreten.
